Notice for families raising children |

Have you completed the Child Allowance procedures? Child Allowance is provided for those who have a residential address within Minato City and who are raising children up to elementary school age until the first March 31 after their 12th birthday. Upon qualification as an eligible household, an allowance of ¥5,000 per child for the first and second child and ¥10,000 per child for the third and subsequent children shall be paid monthly. According to the revision of the related law in April 2007, all infants under 3 years old are subject to ¥10,000 monthly allowance. Application is required in order to receive the allowance. Please refer to the table and apply if your income is within the limit stated. When a child is born Please complete the initial application when your first child is born. If you are already receiving the allowance and a second or subsequent child is born, you must complete the application for raising the allowance. The amount is not raised automatically without this application. Take necessary procedures immediately Child Allowance "Jido-teate" can be received the month following the month in which the application is completed. Apply within 15 days of moving into Minato City or birth of a child. . Have you submitted the Current Status Report "Genkyo-todoke"? Minato City sent Genkyo-todoke forms out in June 2007 to those who have been receiving the Child Allowance since 2006. If you have not submitted the form, please do so immediately in order to receive the Child Allowance in and after June. If you do not submit the form for two years, you shall lose your entitlement to receive the allowance. Process of the initial application Submit the following documents and predefined application form to the Health and Welfare Subsection, Residents' Life Support Section of the regional city offices. 1. 2007 Certificate of taxation "kazei shomei-sho" for Child Allowance
Not required for those who lived in Minato City as of January 1, 2007 2. Certificate of Pension Plan Membership "nenkin kanyu shomei-sho"
Note that a copy of the Health Insurance Certificate is also accepted instead of the Certificate of Pension Plan Membership if you are a member of Kenko-hoken Kumiai, Kyosai Kumiai, Shakai-hoken Jimusho, or Zenkoku Doboku Kenchiku Kokumin Kenko Hoken Kumiai. 3. A document to identify the bank or post office account in the applicant's name
* Not necessary if it is stated in the application form.
* Application can be submitted by mail.
* Application form and Certificate of Pension Plan Membership are also downloadable from Minato City Website.
* Documents other than the application form are accepted at a later date.
