Minato City Subsidizes Entirety Vaccination Fee of Influenza (A/H1N1)

Minato City subsidizes entire amount of vaccination of influenza (A/H1N1) for the following priority group.

Eligibility: Minato City residents and foreign registered residents in Minato City.
(1) An expectant mother
(2) A person who is one year-old and over and has underlying disease such as asthma
(3) A person who is one year-old to 18 year-old
(4) A guardian who has a baby 12 month-old and under or is high immunity deficiency
(5) A person who is 65 year-old and over
Minato City’s households who receive the public livelihood assistance and tax-exempt households, etc. other than above.
(1) Households who receive the public livelihood assistance
(2) Tax-exempt households

A mount of subsidy: First dose: ¥3,600 and second dose: ¥2,550
*in case you receive the vaccinations from different institutions: ¥3,600

Deadline of the subsidy: Wednesday, March 31st. *not later than this date.

How to receive the subsidy:
Minato City’s households who receive the public livelihood assistance and tax-exempt households, etc. other than the priority group: please see the chart below

[When Minato City’s households who receive the public livelihood assistance and tax-exempt households, etc receive vaccination inside/outside the city]

Please pay the vaccination fee at you medical institution to be vaccinated. After this, receive the bill of receipt and the certificate of immunization of the Influenza (A/H1N1)

Please fill out the necessary information in the “Application Form for Subsidy of Influenza (A/H1N1)” The application form is available only Japanese.

Send the receipt, the certificate of immunization, and the application form to Public Health and Disease Prevention Section of Minato Health Service Center
*Copy of the receipt/certificate is acceptable.

<Note> Households of residents’ taxes exempted that moved in Minato City on or after January 2, 2009 are required to attach all households’ certificates of non-taxation of the former address.

The subsidy will be made a payment at a later date.



*The application form must be arrived at Minato Health Service Center by Wednesday, March 31, 2010.
* In case, households who receive the public livelihood assistance and hardly to pay the vaccination fee, bring whole documents including a pre-examination paper which Minato City sent to designated medical institutions. You can receiver the vaccination free.

The government has a strategy that subsidizes entirety vaccination fee of influenza (A/H1N1) for households who receive the public livelihood assistance and tax-exempt households, etc in each city/town. Minato City also has a decision which the reduction of incidence is necessary then increases the number of people who are eligible for free vaccination.
