People with Diplomatic or Official Status Working at Embassies in Minato City

Minato City issues a Vaccination Ticket with Prevaccination Screening Questionnaire instead of usual Vaccination Ticket to people with diplomatic or official status working at embassies, consulates, etc. in Minato City, including diplomats, consular officials, and employees of international organizations.
This differs from the “Vaccination Ticket” normally issued by Minato City and can only be used at mass vaccination sites in Minato City. You cannot be vaccinated outside of Minato City, including at the Self-Defense Forces Large-Scale Vaccination Center in Otemachi.

1. Application

Enter applicant’s information in the List of the Persons to be Vaccinated for COVID-19 provided by the Ministry of Foreign Affairs, and submit it from embassy, etc.

2. Submission method
   Submit via email or from CD-R. Prepare submitting data in Excel. By mail or in person, submit by Excel data. Submitted data that is not in Excel and information that is submitted only on paper will not be accepted.
