Part II What is My Number Card?

My Number Card is a card containing an IC chip, your name, address, date of birth, gender, My Number and a photograph.

1. Available services with My Number Card

• Use for identification
• Online applications, such as e-Tax
• Receive various certificates such as a residence certificate through the convenience store certificate issuance service or the City’s electronic application service

2. Can also apply for a My Number Card using smartphone

Apply for online issuance of a My Number Card by using the application form ID. An application form for issuance of My Number Card with an application ID is required, application forms can be issued at the Counter Services Subsection of the Residents Support Section at each Regional City Office. My Number is confidential and it is not possible to know My Number from an application ID.
