COVID-19 vaccination certificate for overseas travel

The City is issuing vaccination certificate to vaccinated City residents, and those who moved out of the City after being vaccinated using a City issued vaccine voucher, who plan on going overseas.

How to Apply

Please apply either by mail or electronically. The certificate will be issued and mailed by approximately a week.
Address: Minato City Hall, Vaccine Passport Issuing Subsection, 1-5-25 Shibakoen
Application and issuance is also available for those who made a reservation in advance at Minato Public Health Center 2F.

Application documents

  1. Application form
  2. Copy of passport
  3. Copy of the Certificate of Vaccination or Record of Vaccination
  4. Return envelope with your name and return address, stamps unnecessary
  5. Copy of personal identification with your return address
