Vaccination Certificate of COVID-19 or the vaccine passport is issued as a vaccination certificate to those who have received their COVID-19 vaccinations in Japan and plan to travel overseas. The vaccine passport contains information in both English and Japanese
Those who are registered residents of Minato City as of the date of their vaccinations who have received their vaccinations in Japan and who plan to travel overseas.
*Staff related with embassies and diplomatic missions in Minato City, and who were issued with the necessary vaccination documents from the Minato City Public Health Center, are also eligible. Vaccinations received in embassies are not eligible for the issuance of the vaccine passport.
By post, online, or in person at the application counter.
*For electronic applications, take a photograph of items 2, 3, and 4 and upload the data.
