Mail the Certificate of National Health Insurance Eligibility and the Notification of Eligibility Information.

A new National Health Insurance Eligibility Certificate (for those who do not have a My Number Health Insurance Card) or Eligibility Information Notification (for those who have a My Number Health Insurance Card) will be mailed to Minato City National Health Insurance enrollees, and can be used starting Wednesday, October 1. If there are individuals in the same household who are subject to both the Eligibility Certificate and the Eligibility Information Notice, these will be sent in separate envelopes rather than enclosed together. The insurance card you have been using will expire on Tuesday, September 30. Please be sure to dispose of the card properly after the expiration date.
