New Child Medical Certificates have been sent

New Child Medical Certificates, which will be used starting in October, were sent in a blue envelope in mid-September to the homes of children currently receiving benefits under the Child Medical Expense Subsidy Program.
If you have not received the new certificate by October, please contact us. The new Child Medical Certificate is light orange in color.

Child Medical Expense Subsidy Program

This program provides financial assistance for medical expenses incurred by children up to the end of the school year when the children turn 18 (i.e., until March 31 following the 18th birthday). When an eligible child receives medical treatment or prescriptions covered by health insurance, the out-of-pocket copayment cost is subsidized. Eligible children can receive treatment and prescriptions without paying the out-of-pocket share by presenting both proof of health insurance coverage and the Child Medical Certificate at the reception desk of the medical facility.

How to Obtain a Child Medical Certificate

To apply for the Child Medical Expense Subsidy Program, please bring documentation that verifies the child’s health insurance coverage to the Residents Support Section, Health and Welfare Subsection, of each Regional City Office.
Applications can be submitted by residents who meet the following requirements:

1. Both the parent/guardian and the child are registered residents and live within the city.
   (For high school students and the equivalent, it is sufficient if only the child resides in the city.)
2. The child is enrolled in a Japanese health insurance program.

If You Visit a Medical Institution Without Using the Child Medical Certificate

If you forget to bring the Child Medical Certificate when visiting a medical facility or if you receive treatment outside Tokyo, you will need to pay the out-of-pocket copayment portion at the medical facility. Within approximately six months, please submit an Application for Child Medical Expense Reimbursement together with the original receipts.
The reimbursement will later be deposited into the bank account of the child’s parent/guardian registered for the Child Medical Certificate.

When Reporting Changes

Please go to the Residents Support Section, Health and Welfare Subsection of each Regional City Office if any of the following applies:

1. Your address, name, or health insurance has changed.
2. The parent/guardian has changed.
3. If you move out of the city, or begin receiving public assistance, and need to return the Child Medical Certificate.
4. In the event that the Child Medical Certificate is misplaced or lost.
