Certificate of Disability

Individuals with disabilities are allowed to obtain certificates that verify that they have a physical or intellectual disability. With these certificates, they can receive public support.

 

Shintai Shogaisha Techo (Certificate of the Physically Disabled)

Shintai Shogaisha Techo (Certificate of the Physically Disabled) is a certificate that verifies that the holder has a physical disability.

Eligible individuals: Those with visual impairment; those with hearing and equilibrium impairments; those with vocal, speech, and eating impediments; those with orthopedic impairments; those prevented by internal-organ disorders from leading normal lives; those with a certain level of disability specified by the Law for the Welfare of Physically Disabled Persons

Required documentation: Diagnostic report and opinion statement from a designated physician, a photo, and your Individual Number Card, etc.

Ai-no-Techo (Certificate of the Intellectually Disabled)

Ai-no-Techo (Certificate of the Intellectually Disabled) is a certificate for individuals with intellectual disabilities to receive various forms of support. Individuals who wish to obtain this certificate should call the following offices to make an appointment. The Ai-no-Techo is divided into four classifications, according to the person’s degree of disability. The classification is determined based on the comprehensive assessment on items such as mental measurements, social skills, and basic living skills, according to age. Inquire what you need to bring to the screening when you make a reservation by phone.

Seishin Shogaisha Hoken Fukushi Techo (Certificate of the Mentally Disabled)

Seishin Shogaisha Hoken Fukushi Techo (Certificate of the Mentally Disabled) is a certificate issued to persons who face limitations in their everyday lives and participating in social activities due to a mental disability. The prescribed diagnostic report and other documentation are required to apply. For details, contact the office below.

* There is no distinction made between individuals based on whether they are institutionalized or live at home, and there is no age limit.
* Seishin Shogaisha Hoken Fukushi Techo (Certificate of the Mentally Disabled) is valid for two years.

Procedures must be completed to renew the certificate at the end of that period.
