National Health Insurance (NHI) Benefits

Various benefits are available to those insured under NHI. Some types of benefit will not be available unless you apply for it yourself. Please contact the Benefits Subsection of National Health Insurance and Pension Section for information procedures and necessary documentation before you apply. Foreign language documents submitted must be accompanied by a Japanese translation.

Medical Benefits

When the NHI card member undergoes a medical examination using the NHI card, the medical treatment costs are reduced. The portion for which the insured is responsible varies by age. You should confirm in advance whether the medical facility you are visiting for treatment accepts NHI.

At or over school age and under 70 30%
70 and older 20%, or 30% in accordance with income and date of birth

 

Reimbursement of Medical Treatment Costs

If you do not have your insurance card with you at the time of treatment for emergency reasons, you should pay the full cost of your medical treatment once. At a later time, you can request a reimbursement for any medical treatment costs paid at the time of treatment.

 

High-Cost Treatment Benefits

Patients may request reimbursement of high-cost treatment charges that exceed the out-of-pocket amount limit for a month. You will receive an application form from the Tokyo Metropolitan Association of Medical Care for Senior Citizens within three to four months of the treatment. Submit the application form via postal mail, along with the required documents.

If you know your medical treatment cost will be high, you may present a certificate of maximum out-ofpocket amount. You will only have to pay the maximum out-of-pocket amount at the time of treatment at a medical facility.

 

Total High-Cost Long-Term Medical Care Benefits

This benefit is paid when the maximum out-of-pocket amounts under both NHI and long-term care insurance exceed the out-of-pocket ceiling. As the high medical expenses are calculated by household, the unit of payment for this benefit is household.

 

Meal Expenses during Hospitalization

When a resident insured under NHI is hospitalized, she/he is responsible for a ¥460 per meal fee. Its remainder is covered by NHI. If your household is exempt from the resident tax, apply in advance to receive the Limited Payment Eligibility/Standard Medical Payment Discount Certificate. By presenting this certificate of maximum out-of-pocket amount/reduced payment with your health insurance card to the medical facility at the time of admission, your expenses will be reduced.

 

Lump-Sum Childbirth Allowance

When a resident insured under NHI gives birth, a childbirth benefit of ¥500,000/ per child will be paid to the family. The benefit will also be paid in cases of a stillbirth or miscarriage if the woman was pregnant for 85 days or longer. Generally, Minato City Hall pays this lump-sum childbirth allowance directly to the hospital.

* If the childbirth expenses exceed ¥500,000, the patient pays the difference when she is discharged from the hospital.
* If the childbirth expenses are less than ¥500,000, the insured may ask Minato City for the difference.
* If the child is born overseas, Minato City Hall will not be able to pay the lump-sum childbirth allowance directly to the hospital. The insured may apply for this benefit upon returning to Japan.
* If you have paid all the childbirth expenses to the hospital by yourself, apply for the lump-sum childbirth allowance at Minato City Hall.
*The amount received by those who gave birth before March 31, 2023 is 420,000 yen.

 

Funeral Allowances

When a resident insured under NHI dies, a funeral benefit of ¥70,000 will be paid to the person responsible for funeral expenses.

 

Traffic Accident Injuries

If you are injured in a traffic accident, the perpetrator is responsible for paying all medical expenses. However, if that person cannot cover all of the costs or is delayed, apply for benefits under NHI. In this case, a traffic accident report issued by the police is required.
