National Health Insurance (NHI) Premiums

NHI premiums are calculated by household. If the head of household or any of the family members enroll in the NHI, the head of household is responsible for paying their NHI premiums as the representatives of the household.

 

How Your NHI Premiums Are Determined

 

The NHI premium consists of three parts: the basic medical care insurance; support funds for senior citizens aged 75 years and over; and the long-term care insurance.
The basic medical care insurance and support funds for senior citizens aged 75 years and over are calculated and determined for all the insured.
The long-term care insurance is calculated and determined for the insured aged between 40 and 64.
If you have lost your job as a result of your employer’s financial situation, or if you are experiencing financial difficulties due to a disaster or other special circumstances, you may be eligible for an insurance premium reduction or exemption. For details, contact the office below.

Insurance Premium Payment Methods

  1. Bank transfer
    The insurance premium can be automatically withdrawn from your financial institution account at the end of each month. This is an easy way to pay as it saves you the hassle of making the payment manually. If you want to use bank transfer, call and ask us to send you the exclusive bank transfer request form. Bring the following when applying.
    Required items: Bank transfer request form, NHI card or payment slip, your bankbook, and registered bank seal
  2. Payment slips
    City Hall sends out two types of payment slips in June. One is a set of individual payment slips for each month from June to March, while the other is a lump-sum annual payment slip for the entire year’s premium.Pay your premiums by the payment deadline using either option.
    Place of payment: National Health Insurance and Pension Section on the 3F of Minato City Hall, each Regional City Office/Daiba Annex, or at any bank, credit union, credit association, Japan Post bank, convenience stores, or mobile cashier
  3. Special collection (withholding from the pension)
    NHI premiums for households comprising insured people aged 65 to 74 will be automatically withheld from the pension benefits of the head of household. However, if the total amount of NHI and the long-term care insurance premiums exceed 1/2 of the head of household’s pension payment, payment of the premium will be made using the payment slip.

If You Fail to Pay Your Insurance Premium

  1. If a payment is not received by the payment due date, a reminder notice or collection letter will be sent to you.
  2. If no payment is received over a year of the payment deadline, the short-term health insurance certificate must be returned and an eligibility certificate will be issued to you.
    * You are eligible to receive healthcare services covered by health insurance with an eligibility certificate alone but you will be required to pay all medical treatment costs up front when visiting a medical facility.
  3. If an NHI account is in arrears for an extended period of time without an acceptable reason, you may be subjected to having your assets seized.
    If you are having problems paying your insurance premiums or if you are concerned that your payment will be late, contact us as soon as possible.
